I would file a report, but there's really nothing to tell them. I don't know who hit me, where I was, what kind of car it was, or anything like that. I did go to the hospital, spent a night in the ER. Have 2 follow up appointments this week and another ct scan on friday.

You might as well report it, if you know the general route you took. There could be some security footage somewhere on the route that might show something. This is a pretty serious crime, so the cops might be inclined to spend the time to investigate it.
Of course it could also mean that you lost control of your bike and fell off, and dont remember that either.

I'm 95% sure that I did get hit judging by how serious the crash was. I wasn't drunk, and I'm a pretty good cyclist and can't imagine how else I would end up in an accident that ends with me landing directly on my face.

Yes... but that kind of damage from a car while still able to ride your bike afterwards?
I've seen bikes hit a curb and bend the crap out of the wheels.

My front wheel is pretty bent up and my derailleur hanger is bent to hell, but it looks like its still ride-able. I could possibly have been doored or something too, I don't really know. My bikes a pretty sturdy cro-moly frame so its not surprising that the frame isn't really messed up.
I guess I never will know exactly what happened, but getting hit is the only thing that really makes sense to me.
