However, I don't think people really understand depression if you think suicide is selfish, especially in cases of serious mental illness.

I completely understand depression. I've been depressed. I'm depressed reading this crap right now. While I've never been to war, I've worked for one of the busiest fire departments in the nation and seen things that people simply aren't supposed to see.
If you don't think that offing yourself, leaving your family to clean up the mess (literally- someone has to clean it up), plan your funeral, pay your bills, handle the rest of your affairs, etc. isn't selfish then you clearly have no concept of what selfishness is.

I think what they are trying to say is that people in that state just aren't thinking about that. Do I agree that it is selfish? Of course. But are people really thinking of what their actions will bring? No I don't think so.

For someone that isn't suffering from a mental illness, probably being relevant. It's not just ambivalence toward others, there are significant mental processes being altered and impaired. Not to mention the extreme sense of distress and pain accompanying many of them. Yeah, it might be ostensibly selfish to us, but they aren't necessarily capable of thinking rationally or normally.
