That's not always enough. Genes play a role too. I'll never forget the 55 year old guy who lived a blameless life -- worked out, never smoked, perfect diet, perfect BMI -- with arteries throughout his entire body shot to hell. It was only a question which was going to kill him first before the age of 60: the heart attack, the PE, or the stroke.

He must have been cheating or doing something wrong. Lord knows nobody dies who diets and exercises properly.

Healthy diet wont help you much at all if you are genetically predisposed to heart disease. Gene pool doesn't weed out those affected, because heart disease kills you long after most people procreate.
I've known plenty of people who were very active, ate a healthy diet (including a few vegans), and had heart attacks in their 40's. A few of them died because of it. Friend of mines father died at 39, he wasn't fat. Her granddad died at 41, not fat either. Both of heart disease. She is 39 and has major blockage and is in danger of a heart attack at any time. Due to her family history, they aren't going to wait much longer, and are going to do bypass surgery soon.
I've had 3 major heart attacks, been eating a very health diet for most of my life (yeah, I had a few years of booze, smoking and pizza), and my arteries are screwed up to hell. 70 to 90 percent blockage in most of them. Anything I do now to help....isn't going to do that much. I'll end up dropping from either a stroke or the next heart attack sometime in the next decade or so. Unless I develop cancer first of course.
Unless someone invents nanobots that can scour the crap off artery walls, a vaccine is as good as we can hope for. You do know that children as young as 3 have detectable plaque build up in their arteries right? And that diet and exercise doesn't change that fact.
