Does anyone have any information on possible ways of replacing character / environtmental models? File formats?

the DAT format is a bit tricky, and importing anything other than EXACT filesize/dimension replacements isn't really easily done.. not to meantion, I'm not sure how AC would handle it.
I believe all the Textures and Models are standard "DirectX" style textures and models.. just encoded into the DAT file.
while I don't want to call it impossible to do, I will say it's not likely currently.

You'd have to rework not only the model, but the textures and the complete animation set as well, so it's not really feasible.

What about putting a flashing 'glow' around the periphery of a character? (red/yellow/blue to signal that H/S/M is low for eaxmple)

Ru tells you, "CHAZ!"
Simply Red tells you, "I am SO not recovering your body!"

Glow is hard, as that means particle effects. With d3dservice we can fake it to a degree, but having an aura-type glow effect would not be feasible.
For example, a sphere around the character is easy, or some type of ellipsoid.

the female models need some big square gaps filled in on their arm joints, and arms could be bigger and less PEG looking.

Actually...
Before TOD... There was a way to make your toon and everyone elses look like a shadow...
Of course Only You could see them that way...
I was told there was a way to make you toon look like a mini lugian too... but I never got a chance to check it out!

did that only involve hooking the CreateObject and modifying the model id?
because if so, that is relatively simple... but I think what Harbinger is talking about is actually updating and upgrading the models of things ingame to be different than they are.. perhaps better.

Well there was obvious work being done on that before ToD. Textures on the ground such as sand/grass were updated prior to ToD. It did change things in the .dat files.
Models of shadows and such, I have actually done that. It worked fine.
They even had a Bill Gates mod for the face.
It can be done I'm sure. Just requires alot of hard work that no one is willing to do.
That's why complaining and "impossibility" instills itself.
