Its just like all the stars at the awards ceremonies, the dresses, shoes, and jewelry is all loaned to them for the event if not outright given to them. Thats' why when they walk down the red carpet they tell everyone "who" they're wearing.

That designer probably gave her 3 dresses, and in return, look how much publicity it's brought them? we're actually talking about them on a video game board!
As a crafter that would love to make a living off of what I make, I would love to give my stuff away to celebrities all day long if they were ever photographed wearing it I'd make a damn fortune.

Woman to woman, Michelle Obama gets Sarah Palin's need for new clothes – but, she is quick to point out: "Whatever we wear, we buy."
I would imagine it's a bit different when you're campaigning versus the FLOTUSA. I could be wrong, but they generally do not wear a lot of stuff more than once, especially for events. Maybe just around the WH doing gardening with kids and exercising, yeah, but not events like SOTU and official dinners, etc.
