It happens when you start doing squats with extremely heavy weights. 	The main reason I made the video was to check my form, which I dint think is too terrible, even on my fourth rep. When it comes down to it, I'm going for that extra rep if I can keep a decent form. 	Your form is good - when doing max squads, you're going to see a little back movement and also some knee flaring (note your last one and the unsuccessful one where you're left knee moves out a bit). ::Edit:: To help with your back, it can help to keep your eyes on the ceiling during the entire lift.
