another thing that can happen is if you use the armorset thing and save a set with a tabard in it. if you have a tabard on, save a set, then later change your tabard if you ever use the macro to switch your armorset the tabard saved will be equiped. I've done this before where i maxed a rep switched to a new tabard but then hit my DPS gear button and didn't realise until halfwya through the dungeon i wasn't getting rep because my dps set had saved the old tabard as part of it.
