I had a cathal blademaster called Manjina that got nerfed not long after my bard friend and I duo'd a full group of albs 
Not sure if it was the guy on hib in /region that appealed after i gave him crap about adding on our fight or the group of albs not taking a liking to getting trashed by someone with such an 'innapproriate' name.
My champion is called Ribs and I used to have the lastname ForHerPleasure but that got nerfed and I was told if i set it as my lastname again I'd get banned.

I had Phukit on classic that got nerfed along with something else I forget
I had the guild name Cunning Linguists on classic too which also got appealed
