Heh...that line from more than a few aircraft mishap reports reminds me of the wording in a report on a flight test failure of the Ground Launched Cruise Missile back in the early 80s, as we were developing them for deployment in Europe. (For anyone who cares about military weaponry, GLCM was the N version of the Tomahawk Land Attack Missile used to such good effect in both Gulf Wars.)
At any rate, the flight test was conducted at Dugway Proving Grounds, Utah. Anyone who has been to that area (or Utah in general) knows the state is NOT flat. This particular test was designed to evaluate the GLCM's terrain-mapping navigation system. Suffice to say, the test ended prematurely when the missile crashed into the side of the mountain. The report--written by a civilian, not by an AF officer, I hasten to add--simply stated "...at which time, the subject missile attempted to traverse a medium for which it was not stressed."
