There is a bug report out on this. The devs say they will not hotfix it.

red taper, hysop, cinabar, Powdered CD, and ashwood.
double click the mortar and pedestal then click the cd (after you take it out).

Okay because theres no way in hell I'm reading 9 pages of replies I'll tell you once and for all how to do it.
Get a baseball bat or if you don't have one, any big blunt solid object will work.
Pull your computer out into the middle of the room
Repeatedly beat the shit out of your comp with the big blunt object.
A round disc should pop out with the words Asheron's Call on it. You may need a new computer after doing this. I know it took me a few computers before I found one that gave me the CD without breaking it.
