Normally, this, unless the book has been made into a tv series/movie, like Game of Thrones. Upon re-reading it after the HBO series, I now hear the actors' voices.

In many cases how can you know what voice is being spoken until the end of the character's line? Until then, what voice does the character have, and is it awkward if a man's voice suddenly changes to a woman's with "she said"? How do you explain the gender changes to yourself to prevent a break of immersion?
These are the questions I want to know. Being as most repliers have claimed to give voices and accents to each character at an expert level, answering my questions should be no problem. I have many more, but I'll start with these.

most of the time, I don't give the characters particular voices, but if it's a kids' book or if the character has a distinct way of speaking (Hagrid from the Harry Potter books or Jim from The Adventures of Huckleberry Finn come to mind), then I'll go ahead and give them a voice that sounds appropriate, in my head.

when I read to our son when he was younger, I changed my voice for the characters

My husband only reads in his own voice, but I make up distinct voices for people, so I was curious what other people do.
Oh yeah, different voices in my head with different accents. You lose much of the nuance if you monodrone it in your own head.
