Of course with any Apple product, both the good and the bad get way over hyped by the media. And I think this is part of the bad getting over hyped.
I don't doubt there are some defective iPads out there getting too hot. If that's the case, those people should get those exchanged or refunded. I feel pretty safe assuming most iPads aren't going to overheat.
I do wish Apple could have reversed the internal components of the iPad though. Most people will hold the iPad in their left hand. And unfortunately it's the left side that warms up and with an aluminum body, that does draw the heat from the internals outwards, making it feel warm in the hands.
Now imagine your plastic phones and tablets that feel warm in the hand and it doesn't have an aluminum body that's good at conducting the heat, so you can probably imagine it's getting even warmer inside of those devices.
Anyway, if the iPad gets warm enough that it's uncomfortable to hold, then it's probably time to exchange it. So far mine hasn't gotten uncomfortable to hold even after gaming or watching HD videos.
