Decal shows an x in the box for Version 1 Plugin Surrogate

I have check marks in all the other boxs on the decal panel except this one. Shows a red x. I've done all the standard stuff, removed decal, deleted the folder, removed the registry entries etc. Just can't get this one to come up. Any ideas?
